Evaluation of educational clinical environment by resident physicians in nine hospitals

Abstract

Introduction: The evaluation of hospital educational environments aims to measure and enhance the development of academic programs for training healthcare personnel. This approach strives to prepare healthcare professionals in a setting that fosters learning and encourages growth in both interpersonal and technical skills.

Objective: Assess the clinical educational environment across nine hospital units by resident physicians within Puebla State Health Services (SSEP).

Method: This was a quantitative, observational, analytical, comparative, prospective, cross-sectional, multicenter study. A total of 528 medical residents participated from August 1, 2023, to January 31, 2024, through a non-probabilistic sampling method. The Postgraduate Hospital Educational Environment Measure (PHEEM) instrument was administered via Google Forms (Cronbach’s alpha = 0.95), consisting of 40 items on a Likert scale evaluating perceptions of autonomy, teaching, and social support. Data analysis involved descriptive statistics using Excel©, and the Kruskal-Wallis test (SPSS 15©) was used to identify differences between groups.

Results: A total of 427 residents in clinical specialties and 101 in surgical specialties were included, with 189 in the second year of residency. Most residents (195) were at the Hospital General del Sur “Eduardo Vázquez N”. The average score was 103 points on the PHEEM questionnaire, with scores of 65.75 in Teaching, 22.20 in Social Support, and 15.78 in Autonomy dimensions. The highest score (129.25) was recorded at the Hospital General de Tehuacán. The Kruskal-Wallis test results were: p = 0.0467889, H = 38.4 for Autonomy, H = 38.39 for Teaching, and H=38.40 for Social Support.

Conclusions: Significant differences were observed in the educational clinical environments among the hospitals studied (comparison value 15.507), with Tehuacán General Hospital achieving the highest score.
